Identification/usernames: In general, first names will be used in all publicly available displays of entries. Entries may be edited at the discretion of The Advocate.

Value of Prize: Value of prize is approximately $7500. Fitting and initial consultation are free.

Taxes: Taxes (INCLUDING ANY INCOME AND WITHHOLDING TAXES) are the sole responsibility of the winner/recipient of the hearing aid, regardless of whether prize is used in whole or in part. Grand Prize Winner will be issued an IRS 1099 form for the retail value of the prize. No substitution, cash equivalent, or transfer of a prize will be permitted, except that Sponsor reserves the right to substitute a prize (or portion thereof) of comparable or greater value, at its sole discretion. In the event that a potential winner is disqualified for any reason, Sponsor reserves the right to award the prize to an alternate entrant, even if the disqualified potential winner’s name may have been publicly announced. All prize details are at Sponsor’s sole discretion. Unclaimed prize will not be awarded. Odds of winning a prize depend on the number of eligible entries received during the corresponding entry period. Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received. Parents/guardians will be responsible for the tax liabilities of minors.

Eligibility: Entries must be 13 years old and older and reside in one of the following parishes: West Baton Rouge, East Baton Rouge, Iberville, Ascension, Livingston, St. James, St. John the Baptist, St. Charles, Jefferson, Orleans, St. Tammany, Tangipahoa, Pointe Coupee, West Feliciana, East Feliciana, Lafayette, St. Martin, or St. Landry.

You must be 13 years of age or older at time of entry to be eligible for this contest. This contest is void where prohibited by law.

Dates for entries are from January 14, 2015 to February 14, 2015. No entries will be accepted after February 14, 2015.

Winner will be decided by public vote on TheAdvocate.com.

The winner will be notified by email or phone. If the winner is unavailable for prize fulfillment, an alternate winner may be selected. If Hearing Health Care of Louisiana or The Advocate cannot find eligible winners for the prize, that prize will not be awarded. 

Winner will be announced on February 16th, 2015 on theadvocate.com. 

Prize must be claimed by December 31, 2015. 

Employees of Hearing Health Care of Louisiana and The Advocate, their respective parents, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible. 

Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest. 

Entrants further grant to and The Advocate the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.

Hearing Health Care of of Louisiana and The Advocate reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.

Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.

UPICKEM and The Advocate are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.

All results posted are unofficial until winners respond to e-mail or phone notification.

This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
